Thank you for inquiring about warranty replacement parts from ADC. In order to more quickly process your parts claim, please complete the steps below.

  1. Complete the required fields on the claim form below and submit to ADC.
  2. Within (2) business days you will receive an email back from ADC's Warranty Department confirming the receipt of your claim form along with a warranty return authorization (RMA) claim number approving the claim. If your claim is denied this will be indicated and explained in the confirmation.
  3. After receiving the confirmation and RMA approval please print a hard copy of the warranty claim. This printed copy is to be placed in the package with the returned parts. Postage for the returned parts is to be pre-paid and is the responsibility of the customer.

    PLEASE indicate the approved RMA claim number on the outside of the package as well as on the shipping label for identification purposes.

  4. The packaged parts including a printed hard copy the warranty claim form are to mailed to the following address:

  5. Please allow up to (5) business days for an ADC warranty specialist to process your claim. If a credit is requested in lieu of product replacement this credit will be applied 5-10 business days after the claim process is completed.

PLEASE NOTE: It is very important that you provide all necessary information on the online warranty claim form and include a hard copy of the filed claim with your return so that we can process your return more efficiently.
All warranty claims received by ADC that do not have a completed online form and a copy of the claim included with the shipment will be returned unprocessed to the owner and the owner's expense.

Note: Replacement parts will be shipped as soon as the returned part, motor label, or image of the damaged parts is received. The use of an image of a damaged part must be approved by ADC's warranty department.
